    Yes, we will submit claims to your primary and secondary insurance companies or workers compensation company.
  2. Does my insurance company pay for physical therapy?

    . Most insurance carriers cover physical therapy however all insurance policies are different. As a courtesy to our patients we will check your physical therapy benefits. We encourage all of our patients to call and check their own benefits as well. Any non-covered visits are the responsibility of the patient.
  3. What methods of payment do you accept?

    We accept cash, credit card (Visa, Mastercard, American Express) and debit card.
  4. Are my co-payments due at the time of service?

  6. If my injury is related to an automobile accident what information is needed?

    At the time of your first visit please bring in your health insurance card, a copy of the declaration page of your automobile insurance, accident claim number, injury date, your prescription for physical therapy and any legal contact information, if applicable. The declaration page from your automobile insurance will indicate if you have medical payment on your policy.
  7. If my injury is work related what information is needed?

    At the time of your first visit please bring in your health insurance card, your prescription for physical therapy, your work information including injury date, compensation insurance information, contact name and number. Also bring in any legal contact information regarding your case, if applicable. We will as a service to you check insurance authorization with your worker's compensation company.
